How to fix lagophthalmos after blepharoplasty

Patrick K. Sullivan

Lagophthalmos is a condition where the eyelids cannot completely close, leaving part of the eye exposed. Lagophthalmos can sometimes develop after blepharoplasty due to swelling or tightness in the eyelid muscles.

The condition is temporary and may cause discomfort, irritation, dryness or muscle tension in the eyelid area. It may also cause potential damage to the cornea if not addressed. To fix lagophthalmos, various treatments and practices can be implemented to restore normal eyelid function after blepharoplasty.

3 tips to fix it

1. Use lubricating eye drops

Lubricating eye drops or ointments can help keep the eyes moist and relieve symptoms of dryness caused by lagophthalmos. These products should be used regularly as recommended by your surgeon to ensure the eyes remain protected.

2. Eyelid exercises and massage

Performing gentle eyelid exercises and massages can help improve muscle function and flexibility. Here are some common exercises and techniques that may be recommended:

  • Eyelid closing practice
Gently try to close your eyes completely while placing your fingers lightly on the eyelids for resistance. Hold for a few seconds, then relax. Repeat this several times to strengthen the muscles responsible for closing the eyelids.
  • Blinking drills
Practice intentional, slow blinking by closing your eyes fully and holding them closed for 2–3 seconds before opening them again. This can help with muscle coordination and reduce dryness.
  • Circular massage
Using clean fingers, apply gentle circular motions around the upper and lower eyelids to stimulate blood flow and reduce tightness. Be cautious not to press too hard.
  • Stretching exercises
If you experience eyelid tightness after blepharoplasty, a therapist may suggest light stretching by gently pulling the upper or lower eyelids outward while keeping the eye closed. This can help improve flexibility over time.

3. Protective measures during sleep

Using a humidifier in your bedroom can help maintain moisture levels in the air, reducing dryness. Wearing an eye mask or taping the eyelids closed as advised by your surgeon can protect the eyes and encourage complete eyelid closure during sleep.

The healing process

If you are experiencing lagophthalmos after blepharoplasty, you may experience difficulty closing your eyes completely due to swelling, tightness, or temporary nerve disruption. Over the first few weeks, swelling typically decreases, allowing the eyelids to regain more natural movement.

As healing progresses, the eyelid muscles often adapt, improving their ability to close fully. Your surgeon may recommend supportive treatments, such as lubricating eye drops, ointments, massages or regular eyelid exercises, to protect your eyes during this period.

Complete healing can take several weeks to months, even while sticking to your surgeon’s post-operative after care program. If the issue persists despite non-invasive approaches, your surgeon might discuss minor corrective procedures to achieve optimal eyelid function.
