How to choose a collagen stimulator provider

Board certification is the starting point. The details below are worth weighing carefully when you're choosing the provider who's right for your goals.

Here's what to look for before you decide:

01

A good provider explains the timeline before you start.

Collagen takes months to build. A provider who sets out the number of sessions, the spacing and when to judge the result protects you from adding more too soon.

02

Conservative dosing matters more here.

Sculptra cannot be dissolved. A careful injector starts with less, reviews you before each session and adjusts as your own collagen builds.

03

Preparation and aftercare should be explained.

The product is mixed with sterile water before use, placed in the deeper layers of the skin, and followed by massage at home. Clear instructions help reduce the risk of lumps.

04

The injector should be medically qualified.

In Sweden, only licensed doctors, dentists and nurses may give cosmetic injections, and the clinic must be registered with the health and social care inspectorate (IVO). Elsewhere, ask about the injector's training and who handles complications.

05

Product, amount and placement should be documented.

You should know which product was used, how many vials and where. That record matters if you see a different injector or plan further treatment.

Before your consultation, prepare this question

“How many sessions do you expect I'll need, and how will you keep the result even?”

A good provider gives a clear plan: a series of sessions, conservative amounts, massage instructions and a review before each top-up.

About the treatment

About collagen stimulators

What are collagen stimulators?

Collagen stimulators, also called biostimulators, are injectables that prompt your skin to make more of its own collagen rather than simply filling a space. The best known is Sculptra, made from poly-L-lactic acid (PLLA), a biocompatible and biodegradable material that has long been used in dissolvable stitches.

Because the effect depends on your body building new collagen, change is gradual: it appears over weeks and months, usually across a series of sessions. Sculptra is mixed with sterile water before use and placed deeper in the skin, where it supports volume and firmness over time. Another product, calcium hydroxylapatite (Radiesse), adds some volume straight away and also stimulates collagen.

What collagen stimulators treat

Collagen stimulators are used to:

  • Restore volume lost with age, for example in the cheeks and temples
  • Soften nasolabial folds and other facial wrinkles
  • Smooth fine lines on the cheeks
  • Improve skin thickness and firmness over time
  • Restore facial volume lost through lipoatrophy, including in people living with HIV
What collagen stimulators cannot do

Collagen stimulators build volume and firmness gradually. They do not:

  • Give an instant result, since collagen takes weeks to months to form.
  • Dissolve if you change your mind, unlike hyaluronic acid filler.
  • Lift significantly loose skin, which needs surgery.
  • Relax expression lines, which is what anti-wrinkle injections do.
What to expect

Consultation. Your injector assesses where volume has been lost and how your skin behaves, explains the timeline, and estimates how many sessions you may need. In Sweden, the law requires spoken and written information about the treatment and a reflection period of at least two days before it can go ahead.

Preparation. Avoiding alcohol and blood-thinning supplements beforehand reduces bruising. Mention any previous fillers or other injectables.

Numbing. A numbing cream is applied, or lidocaine is mixed into the product, so most people feel only pressure or mild stinging.

Injection. The product is placed with a fine needle or cannula into the deeper layers of the skin. A session usually takes 30 to 60 minutes. Most people have two to four sessions, four to six weeks apart, though your plan depends on your goals and the degree of volume loss.

Mild swelling, bruising or tenderness is common for a few days, and most people go back to work the same or the next day. There is no dramatic change straight away: the result builds over the following weeks and months.

  • Massage the treated areas as instructed. One practice in our network uses five minutes, five times a day, for five days
  • Avoid strenuous exercise for a day or two
  • Protect your skin from the sun

Review. Your injector reviews you before each session, so the amount can be adjusted as your own collagen builds.

Results and duration

Results are gradual. Some people notice a subtle change within a month, with the full effect usually about three months after the final session.

Results commonly last up to about two years, and some practices report longer; it varies with age, metabolism and lifestyle. In the clinical study behind the FDA's approval for cheek lines, results lasted up to 24 months. Touch-up sessions every 18 to 24 months are common to maintain the effect.

Risks and side effects

Most side effects are mild and settle on their own. Because the product cannot be dissolved, careful technique matters:

  • Swelling, bruising and tenderness, common, and usually settling within a few days.
  • Small lumps, small bumps under the skin can occur and usually settle with massage and time. Occasionally firmer nodules form later and need treatment.
  • Cannot be dissolved, unlike hyaluronic acid filler, Sculptra cannot be reversed with an enzyme, which is why conservative dosing matters.
  • Vascular occlusion, rare but serious for any injected filler, when product blocks a blood vessel. It can damage the skin and, very rarely, affect vision, and needs urgent treatment.
  • Infection, uncommon, and treated promptly by the clinic.
  • Allergic reaction, rare. Tell your injector about any allergies, including to lidocaine.

When to seek medical attention

Seek care promptly if you notice:

  • Pale or white patches of skin, or a dusky, mottled color near the injection site
  • Pain that is severe or getting worse rather than settling
  • Any change in your vision, which needs emergency care straight away
  • Spreading redness, warmth or a fever

Contact the clinic that treated you straight away, or seek emergency care, if you notice any of these.

How is Sculptra different from filler?

Most fillers add volume straight away by taking up space. Sculptra works by prompting your body to make new collagen, so the change is gradual and tends to last longer. Hyaluronic acid filler can be dissolved; Sculptra cannot.

How many sessions will I need?

Most people have two to four sessions, four to six weeks apart. The number depends on how much volume has been lost and the result you want.

When will I see results?

Some people notice a subtle change within a month. The full result usually shows about three months after the final session, as collagen builds.

How long does it last?

Commonly up to about two years, and some practices report longer. It varies from person to person, and touch-ups every 18 to 24 months are common.

Why do I need to massage my face afterwards?

Massage helps spread the product evenly and reduces the chance of small lumps. Follow your injector's instructions; one practice in our network recommends five minutes, five times a day, for five days.

Is it painful?

Most people describe mild pressure or stinging. A numbing cream is applied, or lidocaine is mixed into the product.

Can it be reversed if I don't like it?

No. Sculptra cannot be dissolved, unlike hyaluronic acid filler. That is why a careful injector starts conservatively and reviews you before each session.

Can it be used on the body?

Some practices offer Sculptra off the face, for example on the buttocks. The FDA's approvals are for the face, and the FDA advises against injectable fillers for enlarging the buttocks, so ask your provider what is approved where you are treated and what the evidence shows.

What else counts as a collagen stimulator?

Calcium hydroxylapatite (Radiesse) gives some immediate volume and also stimulates collagen. Your injector can explain which product suits the area and the result you want.
